How to use this
On paper a customer answers everything whether it applies or not. Here the form only asks what matters, so a non‑DOT customer never sees the DOT questions.
Try it — answer a few questions as a customer would and see what it asks next. Worth checking it asks the right things at the right point.
Change it — reword anything, add what is missing, mark what should come out. Add a hint to explain a question to the customer, or just leave me a note on it.
The spreadsheet — what you would get once someone finishes. One row each, one column per answer. Switch off what you would not use and add what you need.
Hit Save my changes when you are done and I will see what you changed.

One row per customer, one column per thing you need to write their policy. Tell me what belongs in it.
Every question is already a column. Untick one and it stays off the sheet. Add a column for anything the questionnaire does not ask for yet. Say what it is in the note and I will work out where it comes from.
